You may either make an appointment by telephoning or by calling personally at the surgery.
Routine appointments are given at the earliest available time.
Appointments are made at ten-minute intervals and are for one patient only – if you have other family members who need to see a doctor please book a separate appointment.
If you feel that your problem is medically urgent please telephone as soon as possible after 8.30am in the morning – the phones are busy so please keep trying.
There is a high demand for appointments -
If you do not attend 3 booked appointments in up to 6 months, without contacting the surgery to cancel, you will be sent a warning letter stating that if this pattern is repeated within the next 6 months you will be removed from our practice list.
If possible please try to telephone reception before 11am if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.
There is patient parking at the rear of our building
Two disabled car parking spaces are available in the car park. There is a bell at the front entrance to summon assistance from the receptionist if required.
All downstairs consulting rooms are accessible, and there is a toilet available for disabled patients.
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.
If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Doctor's sick notes can only be given for an illness/injury that has lasted longer than a 7 day period, which the patient has self certified for.
To request your first sick note you must discuss your ongoing problem with one our GP's or Advanced Nurse Practitioners.
If you find your problem is ongoing and your sick note has expired you are able to complete and return the below form, where it will be passed to a GP for review. In some cases the GP may request you to book a routine appointment with them to discuss.
