Dear All Patients,
Please be aware we are undergoing refurbishment works from Monday 1st February for 10 weeks, during this time there will be no access to patient facilities.
There will be some disruption to the surgery although we will be operations as usual.
Please do not come to the surgery unless you have an appointment.
We apologise for any inconvenience caused and thank you for your patience during this time.
The Covid Vaccination Hub at Rosebank Surgery is now up and running and working well with other Practices from the local area. As a group of Practices working together we have vaccinated over 3000 patients.
As of today Rosebank Health has vaccinated all of our elderly care home residents and care home staff, over 600 80+year olds and over 400 75+ year olds. We still have 300 over-80 year olds to contact and they are our current priority to get vaccinated. We have staff working throughout the day phoning and inviting these patients to our next clinic, nobody has been forgotten!
Unfortunately we do not have any control over the deliveries of the Covid Vaccine, however we do get told when and how many will arrive but at very short notice. Gloucestershire is way ahead of many other areas.
PLEASE DO NOT PHONE THE SURGERY REQUESTING A VACCINE! We would ask you to wait patiently until you are contacted, we have to follow the Government guidelines and offer to patients as per the cohorts. Telephone traffic into the surgery is very high at the moment, please do not ring the surgery unnecessarily as you are stopping patients who may have an emergency.
Please be mindful that not only are we working incredibly hard to provide our normal services, we are part of the largest vaccination programme the NHS has ever seen and we also have staff who are suffering with Covid and isolating! We are doing our best!!!
To register for our online services you will need to complete the below form. We will then contact you and let you know how to provide your two forms of identification. Please do not attend the surgery.
Once you are registered, we will issue you with a username and password, and you will be able to use the service to:

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
For any request for letters/medical records, please complete the form below and return via email to Bartongate.firstname.lastname@example.org
Request for Medical Information
There may be a charge for any letter requests where we will require payment in advance. In these cases we will inform you of the payment details.
Whilst we are aware that some patients might understandably be anxious about the Government’s recent announcements around the use of face coverings / masks in various public settings, the practice are not able to issue any letters or statements requesting that patients be exempt from wearing mandatory face coverings.
The Government guidance on exemptions suggests there is no requirement for evidence for exemption, therefore it is sufficient for an individual to self-declare this.
Details on people who do not have to wear a face covering can currently be found at www.gov.uk/guidance/coronavirus-covid-19-safer-travel-guidance-for-passengers#exemptions-face-coverings
NHS Travel Vaccinations are offered at the Practice however patients are required to complete and return the patient Travel Vaccination form. You can find this attached below or obtain a copy from reception at the Surgery.
Travel forms are required to be returned at least 8-10 weeks before traveling. If the departure date has not been filled in the form then we will not be able to accept or offer an appointment.
Please be aware that we only have a select number of available appointments to offer. Due to this we currently have a waiting list so we may not be able to offer an appointment before travel.
Please note that we only offer NHS travel vaccinations and should you require any privately it is the patients responsibility to source themselves.
Travel Health Questionnaire
The below form is required to be completed and returned to the surgery at least 8-10 weeks before travelling.
